Head Official’s Report Form

End date of the event you are reporting. If you are reporting a one day event, leave blank.

List the races included in this report. Examples: Sprint Triathlon, Standard Triathlon, Sprint Duathlon, Long Course Aquabike, etc. You can report several races on the same form. If required, you can also report any race separately on a separate form.

List your qualified secondary and moto officials and the positions they filled. You can add lines by clicking the plus sign (+) to the right.

List your volunteer, in-training, and job shadowing officials and the positions they filled. You can add lines by clicking the plus sign (+) to the right.

List any officials who volunteered or filled in on an ad-hoc basis and the positions they filled. You can add lines by clicking the plus sign (+) to the right.

Violations are reported only if they result in a time penalty, athlete sanction, DSQ, protest, accident, or incident. Stop-and-correct violations are not reported unless they result in one of the above.

Protests and appeals are reported only if they are correctly filed using the protest form, with payment, within the allowed time. If a formal protest is filed it may be possible to resolve it informally and have it withdrawn without convening a jury, in which case report the circumstances and the resolution but return payment. If a competition jury is convened, the payment is returned only if the protest is successful.

Accidents and injuries should be reported if they result in a DNF, serious injury, or removal from the race site by medical personnel. Additionally, accidents should be reported if in the opinion of the head official, they could result in a serious injury being reported post-race.
